I've always been fond of the "external enclosure" look. Put the rad on top of the case, lengthwise (longest dimension parallel to the case's longest side). The average case is just wide enough to fit a car heatercore with two fans on either side in a push/pull configuration, and a shroud between each fan and the core. Mount it that way, and you can use a radiator long enough to take 2x 120mm fans on each side. Can look nice and symmetrical, and you can run the water lines through a modded 5.25 bay or through a pci slot(s) if you don't want to cut much metal.

A heatercore should run you about twenty dollars (US). There's lots of info about them on the forums here.
