It takes up 2 PCI slots, which only the GPU block does anyway so adding a heat spreader won't cause any extra problems. The heat spreader is epoxied to the RAM and GPU, the waterblock is bolted straight through both heat spreader and card.
Adding a peltier would be very easy if longer screws where used.

Yes the GPU was lapped together with the first layer of copper so they should be on the same level before adding the second layer.
I used 1 mm copper.
