Just get a pipe wrench on the tool holder that should losen it up.
It will leave teeth marks, but should still deem the tool holder useable.

Mine wont hold 3/8 shank either, and I do need to get some tool holders like U have.
I have been trying to bribe Pauls brother to make me some.
