Don't know exactly (using the CD = 120mm method at the mo) but I'd say 250mm x 200mm or 10" x 9" approx. I don't want to scratch the paint off at all.

It came with a fan and shroud already mounted. Can't tell much by it other than it's made by Delanair, part# D201/387A serial# 0000003 (3rd one ever!!??) @ 24v

Just tested it at 12v, man is powerful. I haven't got my dB sound meter here but I'd say around 36-42dB, not bad for a dirty old fan in need of a strip down and rebuild.
