Reinstall your OS (from assist, not GUI or from an image)

To me that an indication of a HD failing. I would remove the HD and copy the first 25-30 meg of data from the drive using DD RAW copy. Instruction in wiki or FAQ's, can find a link in my signature. You may have a HD failing and you will need this img to reload a bare drive. I would then run SpinRite and see if it can repair the drive. If that does not work try Re-loading the OS.

I just upgraded the drive. Both the original and the new drive gave me the same results.

Time to reload the OS, like Andy suggested. It needed to be done through assist inorder to repair corruption.

I tried to download another copy of the OS from adaptec but they tell me that my server is not "entitled" to the download.

What is the MD5 check sums. Here is both the sup and updater. This will tell us if you have a damaged file.
